  • Leaving methadone treatment: lessons learned, lessons forgotten, lesso...

  • Abstract: Despite the demonstrated benefits of methadone maintenance, there have been concerns about the ethics, necessity and expense of maintaining addicts on methadone indefinitely. The inability of many patients to achieve normative levels of psychosocial functioning with methadone, combined with widely held attitudes favoring drug abstinence over replacement medication, has led to attempts to promote time-limited methadone treatment. This paper reviews the published research literature on post-discharge outcomes of patients exiting from extended methadone detoxification, "abstinence-oriented" metha...

  • Health care reform: lessons from the past, lessons for the future.

  • Abstract: Health care reform is a perennial issue in elections--partly because of the challenges facing health care and especially because health care is an important issue for the swing voters. Making reform happen is harder. There are deep divisions within each of the parties--not apparent during elections. Bipartisan support--and the willingness to accept the best legislation that can be passed as "good enough"--will be key.

  • Abstract: In this paper, we analyse Canadian health promotion discourse past and present, in the context of selected federal and provincial government policy initiatives. Principally, we examine the health promotion discourse articulated in A New Perspective on the Health of Canadians, Achieving Health for All: A Framework for Health Promotion, the Ottawa Charter for Health Promotion, Improving the Health of Canadians, and Canada Health Action: Building on the Legacy-Volume II-Synthesis reports and Issue papers. We argue that the health promotion lessons of the past 30 years contained within these repor...

  • Permanent prostate brachytherapy: lessons learned, lessons to learn.

  • Abstract: Current techniques for permanent prostate brachytherapy are associated with excellent biochemical control in patients with localized prostate cancer. Data now available confirm 5- to 10-year results with this treatment modality that are comparable to those with external-beam irradiation or radical prostatectomy. Nonetheless, treatment protocols and techniques for prostate brachytherapy vary from center to center. Controversy exists regarding the selection of patients for brachytherapy alone or in combination with external-beam irradiation. The role of neoadjuvant androgen deprivation also rema...

  • Promoting Instead of Controlling Health - Lessons Learned, Lessons to ...

  • Abstract: Health promotion, as laid down in the Ottawa Charta by the World Health Organisation, embraces the call for self-responsibility. Self-responsibility, however, is not merely directed towards apparently personal risk factors. The Ottawa Charta clearly puts health in a wider societal context and follows the line of early social medicine, which, as Virchow put it, understood politics as medicine on a large scale. With the increasing dominance of neoliberal thinking during the 1990s, this view of health in its societal context was pushed aside to make way for an individualistic understanding of sel...

  • Applying pharmaceutical lessons: learning lessons from the past.

  • Abstract: Systematic evaluation of a project, product or other successes, and failures, is an important method for improving the performance of a company, as well as that of an individual. One should evaluate and determine if lessons learned can be applied to the systems and procedures being used in the company. Even if the company does not have to adjust its strategies, procedures, portfolio or systems as a result of lessons it has learned, there may be pointers to communicate to staff. It is easy for many aspects of a system to become outdated, particularly in an organization that is growing or changi...

  • Toward the twenty-first century: lessons from lead and lessons yet to ...

  • Abstract: A consideration of recent research on lead is instructive for developing research strategies in environmental toxicology. Lead research has demonstrated fruitful interactions between clinical and basic science. Thus, while epidemiological studies have suggested that central nervous system (CNS) effects in children are observed at the lowest increments of lead exposure, basic research has elucidated some of the molecular events that underly this lack of threshold at the neuronal level. Similarly, clinical studies indicate that early exposure to lead produces functionally irreversible damage to ...

  • Abstract: Gastrin is the principal hormonal inducer of gastric acid secretion. Chronic hypergastrinemia, leading to hypersecretion of gastric acid and increased proliferation of parietal and enterochromaffin-like (ECL) cells, has been well described. In contrast, the physiological consequences of chronic gastrin deficiency had been poorly understood until the recent genetic engineering of mouse mutants containing a gastrin gene deletion by homologous recombination in embryonic stem cells. This themes article describes the consequences of constitutive gastrin deficiency on the development and physiology ...

  • Lessons from genetically engineered animal models. VII. Apoptosis in i...

  • Abstract: Apoptosis plays an important role in homeostasis of intestinal epithelia and is also a stress response to toxic stimuli. Transgenic and knockout mice have provided insights into the regulation of intestinal epithelial apoptosis that could not have been obtained by cell culture techniques. Two broad types of apoptosis have been characterized: spontaneous apoptosis, which occurs continuously at low levels in the normal, unstressed intestine, and stress-induced apoptosis, which occurs after genotoxic insult such as exposure to gamma radiation or DNA-damaging drugs. Spontaneous apoptosis occurs at...

  • Abstract: Life's Journey If life is indeed a journey, then poetry must be the map that reveals all its topographic possibilitiesellipsis while science is the compass that keeps us from getting lost. -R. Initial palliative treatments for Alzheimer's disease (AD) have proven beneficial and have gained FDA approval, the use of animal models for studying AD and other neurodegenerative diseases has achieved wider acceptance, and importan...
