  • Abstract:  The phrase "working downhill" refers to recognizing a person's underlying motivations, rather than assuming that a certain mode of behavior is appropriate for all people. One way to counsel a patient with HIV is to work downhill by understanding the patient's perceptions of HIV, and counseling within that framework. To communicate with patients of different backgrounds a therapist should address the inherent beliefs of a patient's culture, while focusing on the patient as an individual. The therapist must also help the patient to overcome instinctive behaviors that undermine HIV prevention.
